Shin Megami Tensei IV: Apocalypse

As many of you are no doubt aware, Shin Megami Tensei IV: Apocalypse is a day away from releasing in North America. The hotly anticipated RPG released in Japan months ago, and Atlus has been busy over the past year getting the game ready for Western audiences. In a rare oversight, though, it seems that the localization team accidentally left in some Japanese text, and it'll be in the game when it releases later this week.

So here's what happened. The Japanese text shows up twice, both times in a very specific context during a certain boss fight at the end of the game. The text is only found if you happen to have your partner knocked out at the moment the boss initiates dialogue. Rather amusingly, the text didn't get caught because everyone on the QA team was too good at the game and never found themselves in a scenario where their partner fell unconscious during the fight. A fix is being looked into, but it's currently unknown when or if the text will ever be fixed. You can find the official apology here on Atlus' blog.
