Mega Man 1 and Ghosts 'n Goblins. The original Contra
I'll give you MM1 and GnG. Contra on the other hand was a breeze for me. In fact by the end of it all i had earned enough lives to be my own Konami code. I once did it for kicks, and on the last boss let myself die 12 times before finally seeing the 4 medals drop to 3. Ninja gaiden 1-3 did it for me in terms of difficulty. 3 was by far the easiest but still wouldn't let me win a particular level in 1 go. It's not fair that a level is so long that their is not enough time to defeat the boss after making it through it.

The Harder the conflict, the more glorious the triumph. When the going gets tough, the tough gets going.
Ikaruga.
I'm still playing it on my Gamecube, trying to finish it on Normal, but as soon as the fifth stage begins, I just die again and again.
12 years later maybe it's time to try it on Easy...

VVVVVV's achievements, because the main game is easy, but without dying it's impossible.
EDIT: Forgot to mention Cosmophony's 5th level is impossible in every way.
FE10 on Hard or FE13 on Lunatic+. Lunatic+ was just unfair as enemies had skills you normally can't get and were all super OP , but FE10 was genuinely hard, like really hard.
Hard to decide. There's R-Type, Super Ghouls 'n Ghosts, arcade Shinobi, Chakan… tough stuff. And that's not getting into games I've only played for a few minutes like Ecco the Dolphin. And arcade games in general are extremely hard.
The toughest game I've beaten is Castlevania III. I haven't beaten the first level in the second quest, and I won't.
Too many games I've never completed to make a definitive answer, but I seem to remember playing the cracked version of Speedball 2 on the Amiga. Super Nashwan were just about beatable if you played with the cheats on to give you unlimited cash. But try playing without the cheats and the 1st Division was impossible.
You guys had me at blood and semen.
What better way to celebrate than firing something out of the pipe?
Honestly, and others who have played the game can back me up on this, the hardest game I've ever played (and quite possibly the hardest game ever made) was F-Zero GX. I'd like to meet ANYONE who could beat the story mode in that game, even on the easiest difficulty setting. If anyone reading this has done so, I salute you!
