For me it was Donkey Kong Country. I was young when I got this for my birthday and it took me 3 yrs to beat that game. I almost broke my SNES controller.
I'm only going to count soemthing I actually tried for this.
It would have to be Ninja Gaiden 2 on the NES. I tired so hard to get past one level, but couldn't do it. I tried entering an invincibility code too, but couldn't do that either lol.
I tried a bunch of kaizo difficulty Mario ROM hacks. I sucked at them, cause damn, they were so difficult as to be ridiculous.
For non kaizo/platform hell games... not sure. The Mario fan game 'Super Mario Country' might be a good one, since I nearly lost my sanity with some of the levels in that way. Way too many overpowered enemies and way too little in the way of items/power ups.
With the amount of games ive played, thats quite a difficult question.
Ninja Gaiden, both the classic and the new trilogy are both up there for sure. The older ones being quite a lot harder due to a severe lack in polish.
Then there are the From Software games liek the Soul Series, Armored Core or Kings Field. All very manageable, sure, but they require loads and loads of finesse, planning, patience and knowledge of the games mechanics.
And i usually lack in the patience department to even aquire the rest of these skills
Also, some SMT games.
Those games are pretty brutal but (technically) never unfair. Same as above.
And then there is everything running on a timer.
The Attelier Series, Persona series (the newer two), Pikmin and even Majoras Mask.
Its not that they are hard per se, but i simply cant deal with it. Call it OCD, but i usually want to finish my game as thoroughly as possible, which isnt possible with a timelimit on your back.
For me, they are almost impossible (less with Majora, i still dont like it ), especially Persona, as there really is just one golden run through with a very very tight schedule that nets you a (somewhat) perfect game.

Some are plenty of ridiculously hard games that are just unfair and unenjoyable. I think the hardest game that I still played and enjoyed its difficulty would be Dark Souls. Incredibly unique game (except for its 2 series brethren)
I don't know.. I have played a lot hard games, so I wouldn't have the time to select one, but if I did it'd be an old game. Mega Man, for one is hard stuff. In fact, most old platforming games...
And let's not forget the Metroid series. I don't know whether it's hard, it's just... [I don't even know the right word here]
Mega Man comes to mind, but that's a game you can get used to after some practice. Depending on your perspective, I'd say Zelda II: The Adventure of Link, since I consider any time I have to look up something on the internet to be a failure. I think long and hard for days before resorting to that, and I had to look something up 3 times in that game!
