Rarely. Only two of the games I got last year actually came out that year and I didn't get them on launch day. The only game coming out this year that I intend on getting on launch day (or maybe the day after) is Animal Crossing: New Leaf. I might have got Shin Megami Tensei: Devil Survivor: Overclocked on launch day if I had a 3DS at the time but even if I hadn't lost my first 3DS a few months before, I might have waited for my birthday due to how close it was to SMT: DSO's release date.
When I had a Job, I'd at least try to buy a game the month it came out, but now with my more limited spending budget, I have to make use of deals (usually on older releases).

The only two games I've gotten on release day are Skyward Sword and Portal 2. I almost never buy games at full price since I can usually get older ones for much cheaper.

Only Japanese games from smaller publishers. I pre-order games like Rune Factory 4, Project X Zone, SMT IV, Etrian Odyssey IV, etc. (wow, a lot of 4s this year...). I don't bother with first-party Nintendo games because those will sell well regardless, and there usually isn't a pre-order bonus for those.
One thing I really like about handheld games is that they're relatively free of the crippling bugs that often seem to affect PC or home console games on release.
Only for games I'm dying over. But that happens a lot!!

At the moment, it's a rare thing for me to buy a game on release day, simply down to funds. However, one exception my wife and I are making is for Animal Crossing. Her as a physical copy and me as a download version.

i limited myself to one game a month so yes I do get some of them on release day. Except for Fire Emblem Awakening. I have Pokemon X & Y, DKCR 3D, Project X Zone, SMT IV and the Animal Crossing bundle on pre-order.
