No, this hasn't happened to me because I get new Games on Launch Day, either from GAME or the MyNintendo site.

Secondhand Games come from either GAME or CEx and I always ask the cashier if I can check the gold chip for damage, therefore I KNOW the game card is in the case.

However, an LCD screen instead of an OLED screen shows there will be a redesign two years after Launch.
