How about the final boss of Earthbound? You don't even know what the heck you're fighting, and the only way to defeat the thing is by prayer. And that music that suggests you aren't going to survive the fight...

The designers actually expected players to try to fight Gyigas the normal way and only find out by using Pray as a last resort (as it's a move that normally is a risky chance (the kids says YOLO these days, I guess). I think one of the possible effects is to kill the entire party, and who's going to risk that on the final boss?
(statistically Gyigas has 2000 HP but it is meant for that number to never change, with fake damage amounts printed just to mislead the player. However I hear poison can do ACTUAL damage to Gyigas but then it glitches the game out if Gyigas dies since the battle is not supposed to end in the standard way)
Surprised to not have seen it, but the Dead Hand. Hands grab on you holding you helpless as this thing slowly shambles toward you so it can chomp poor young Link.
First party Nintendo stuff? Definitely those creepy soulless replicas of yourself you create with the elegy of emptiness in Majora's Mask. Or the final hours of any three day cycle. Or that poor girl's father in Ikana Valley. That game is so dark and grim, and I love it so much.
There's quite a bit of creepy stuff through the years but to name a more recent one, in Game & Wario when the mom first climbed out of the TV, walked around with the crazy music playing, then sits down and watches TV while you're supposed to resume your minigame playing like nothing is going on, just waiting for her to turn that thing off and look right at you with that big empty expression.

That scene with Ilia, the Triforce and the Dark Links in Twilight Princess is borderline creepy for me... I also remember getting a jump scare when Yeta shifts to Blizetta.
Other than than that, Gyigas in Earthbound is disturbing. I would not say I got scared, but really anxious playing that fight.
Oh, and when I was a kid, Lavender Town and the Pokémon Tower in Pokémon Red gave me goosebumps.
SA-X. That thing made me panic so much on my first playthrough of Metroid Fusion. I'm still afraid to drop down in that one room where there's no indication that it's right below you waiting for you.

Redeads on Wind Waker aren't frightening because they are redeads. They are frightening due to how much the break the entire mood of the game. Wind Waker is such a bright and happy and colorful experience for most of the game. Suddenly you're just exploring the next dungeon still keeping that mindset and then all of a sudden REDEADS EVERYWHERE. Compared to the rest of the game these are nightmare fuel.

I don't know about anyone else. But the Sunday-Monday 3 am Alien message on the Tv creeps the shit out of me, especially if it is at 3 am in the dark.. shudder

But seriously, I don't find much stuff in Nintendo games creepy. The only game (which I can remember) that made me shut off the system, head back to bed, and wish I never played the game in the first place... was Trauma Centre: New Blood. The level with the nun and the collapsed church foundation... and the lady's he-- UGH. And it didn't help that I was playing the game in the night... X(
