Yes NGIII is the hardest and one of the most intense videogames I've ever come across. It's also hands down the most difficult of the NG Nes trilogy. And you know what? I managed to beat it which was around 4 months ago. It took lots of time, patience and frustration but I did it and it had to be of been one of my greatest gaming achievments of all time . It was like the feeling I got when completing Wario Land Shake it! 100%(actually, THIS was the hardest and most insanely thing to achieve out of all of the videogames I've played in my life) but that's another story all together. Seriously though, if you can beat NGIII than I give you mad props because if so than you've seriously got some gaming Master Guru skills None of the Mega Man games can touch the difficulty of the 3 Nes Ninja Gaiden Games. Castlevania III: Dracula's Curse is the second hardest game I've ever beaten. But again nothing can come close to the insane skill based brutality of completing all of the obstacles in Wario Land Shake it! It gave me nightmares.

And damnit, I remember almost finishing the oh so annoying and frustrating 'TMNT' for the Nes...I mean that game is just so unpredictable with it's cheap enemy glitches and respawning and poorly done Levels, that I just couldn't do it. Although I did make it to the final Level right before Shredder. Yet I called it quits after a few Tries. Another tough as nails videogame is Blaster Master for the Nes...I can't beat the Finall Boss is in his Final Form. And I don't think I'll ever will try again ;p
I just beat Mega Man X today, actually. I don't really play many difficult games, so that's the hardest one I've beaten. I like games that are big on story, not difficulty. I've played Mega Man 1-8, and I can't even beat those ones.
Megaman 2 & 3. Very, very hard. Also personally found them quite boring. But I bought them so I figured I should finish them.
Boring? MM2 Hard? how dare you speak such silly words. Nahh but to each their own. I can beat both those games in my sleep...Then again, those Wily Sub Levels in MM3 can get tricky and those Fake MM2 Robot Masters were a pain in the ass. MM2's only difficult part for me are the Wily Castle Bosses.
I'd have to give it some serious thought.. but also off the top of my head I'll say Orbient. To complete that game 100% i.e Every moon on every level... well, took me years (admittedly I had periods when I ignored it and played other things... but still took me forever)
Majora`s mask. That game is like bootcamp. If you can beat that, you can beat ANY zelda game.
