It's not actually Nintendo but the Mayday boss and Rachael in Resident Evil Revelations really scared me.

The ship captain boss also creeped me out. He was gross and had a OHKO attack! D: Racheal didn't scar me though. Just a chick with big boobs, long tounge, and claws. And pleeeenty of cleavage.
Racheal didn't scare me as much as the ship captain, but it scared me when she said FOOOUND YOU and when she was moaning as you 'killed' her. Also the final boss was really anticlimatic... I didn't remember ever seeing him previously in the game.

Also, there are a couple who I forgot to mention earlier:
*Dark Matter - I fought him at the end of Kirby Quest in Kirby Mass Attack. His original form wasn't too scary, but then, you think you've got him beat. And then he comes back as this creepy giant eyeball thing! *The Living Gate - I don't know what else to call it, but it's the gate that gets up and flies around at the end of SMB2. Those gates were creepy enough, let alone when they jump off the wall and attack you!
Used to think Andross in his robot form was kind of creepy. Another one is Majora's Wrath from Majora's Mask. (The last form of the last boss)

1. Brawl Doll (Wario World) - Crazed, large puppet baby angle 2. Clown-a-Round (World World) - A boss that has a psycho laugh and the ability to remove its many faces. I can't forget the giant circus ball bomb either. 3. Winter Windster (Wario World) - A dark blue dude with a little body but massive head with piercing blood red eyes. 4. Zeta Metroids (Metroid 2) - Found these to be the most difficult of the Metroid evolutions (excluding the Queen) and it is truly terrifying to deal with in a number of the arenas. 5. Man-at-legs (Pikmin 2) - Lord help your Pikmin army when this mechanized predator has its laser pointer machine gun aiming for them. 6. Waterwraith (Pikmin 2) - Undetected in a number of the sublevels in the hole this boss first appears in, the Waterwraith becomes even more of an oddity with its habit for surprise landings. You had better run if you're anywhere near its concrete rollers, and always have your Pikmin in a safe hiding spot. 7. SA-X (Metroid Fusion) - Not so much the encounter itself, but the tension this antagonist provides just walking through cooridors nearby and in chases. Its mutated form is also a disturbing site. 8. Crayk (Legend of Zelda Phantom Horuglass) - An giant freakish crab with the ability to turn completely invisible as it charges Link. It was startling the first time I enountered the situation, and needless to say, I found myself wanting hearts. 9. Dead Hand (Ocarina of Time) - Bloody, pale white zombie blob with a zillion arms ready to grab me in a well brimming with sinister history...Compared to the game prior to the bottom of the well, that's a very disturbing enemy.

Medusa in the original Kid Icarus. Reaching the end of Kid Icarus, I expected to see a Medusa looking something like she did in the instruction book. THAT THING was the last thing I expected. I guess I was scared because I was startled. Being startled can be very startling! That and the Big Bot toward the end of the Great Palace in Zelda II were memorable scares for me. Did anybody else get a serious scare from Medusa the first time? I mean come on, nobody could expect THAT! Even now it still looks creepy. I'll definitely be more guard when I reach the last boss of Kid Icarus: Uprising!
Also, the constant fear of running into SA-X in Metroid Fusion definitely makes it a scary boss! It was a bit paranoia-inducing!

@i-U: Oh yeah, forgot about the Waterwraith...I kinda freaked out when I first saw it, seeing how I couldn't kill it at the moment. xD
Another I should add, wasn't exactly scary...but I did get chills down my spine when Deviljho spotted me the first time. (Tri) Also, not really a boss, but sheesh...I STILL jump out of my seat from that stupid Mad Piano. xD
If we're naming MHT monsters, Alatreon. That boss makes me tremble the whole fight, though it's less that he's scary and more that he's super powerful and I don't wanna die and look like a noob

Mimi didn't scare me too much on her own, but having to run away from her while she was stalking you did. I thought the first Mimi fight was ki.d of funny.
