Since we're posting pics, Kid Icarus instruction book comical-looking Medusa vs. unexpected final boss Medusa that was at the time one of the scariest looking bosses around.
Unexpected Zelda II giant bot that drops on your head in another location than the one shown...
Everything about the bottom of the well and the dark temple from ocarina of time is still as scary as it was when I was a kid. And those dead hands, wall masters and floor masters... I hate them!... Not to mention redeads... Bongo bongo is cool.
A boss that kind of made me flinch was blizzeta from TP and her sudden "BLEEEH!" scene. Majora's Wrath is kind of disturbing with her devil eyes and stuff. That golden giant from skyward sword too...
OH I know, that battle with Bowser to save Peach in SM3DL was quite scary at times because I was mini Mario.
I'm sure there's more but I can't remember them rigth now.
Dead Hand: Hands down scariest boss ever Most ing fights in MP2:Freaky Zelda's enemies are creepier than bosses. Wind Waker scared me. OOT's Shadow Temples are why I won't beat it again. HATED THE SA-X GIYGAS IS A LIVING NIGHTMARE Kirby bosses are the exact opposite of cute. Nintendo: The family friendly company to give kids nightmares. Oh and the Shroobs Fawful

Phantamanta (Super Mario Sunshine)- Maybe not particularly scary, but still intimidating.
I didn't dare to try this level when I was kid. For some reason, it scared me to death. Also Nightmare from Metroid Fusion is pretty.. weird mostly, but in a bad way.
