Whatever random system Nintendo has in place that decides what puzzle pieces you get when you spend playcoins on them.
I don't even have half of the pieces collected yet, yet it somehow comes to the conclusion to give me a piece I already have 60%-70% of the time. What's worse, is that I somehow manage to get the same piece (usually one I already have) 3 times in one sitting, and this happens EVERY time.
I know they're trying to encourage you to get the pieces by actually streetpassing, but still...

Never trust Random Number God, he/she always screws players over.
Thinking about open world games, being forced to drive in environments that clearly aren't built for fun driving has always been a pet peeve of mine. Grand Theft Auto 4 comes to mind - absolutely hated driving in that game.
Pushing things into place in environmental puzzles - the older I get the more I hate this thing, especially when said things are heavy and force you to move at snail pace.
Hard Mode that gets unlocked only after you beat the normal difficulty level.
Having to continuously go back to a certain stage only to get a power-up that is essential to do something specific in another stage - see Super Mario 3D World.
I do accept QTEs but only as long as they don't involve movements with the sticks and/or pressing down on a single button repeatedly like an idiot.
Jet-Packs that allow your character to stand still in mid-air. I'm not talking about devices that boost your jumps like in Wario or Cave Story but real jet-packs that make your character float in mid-air even when it stands still. My brain can't comprehend how this mechanic works for some reason. Dark Void Zero was unbeatable for me.

Excessive hand-holding. This is really bad in Skyward Sword, among other titles. The game has plenty of puzzles, but treats you as if you're brain-dead otherwise. They even build it directly into the game, with Fi pointing out things that are blindingly obvious most of the time.
While Zelda's not the only game guilty of this, it's the worst offender I've played in some time.
Oh Pokémon X & Y... I hope the gen 3 remake doesn't do this, and stays true to the original game.
One of the worst to come out of gaming is DLC, as much as I hate to say it. I love good DLC, but when games are being produced anticipating DLC sales, it takes from the actual game's content. DLC should be free, IMO. I wouldn't have a problem with paying up to $80 for a new game if it was getting good, free DLC for at least a year.
I don't mind DLC so long as its treated like a side story or expansion pack, and only worked on when the game is in post-production. That's largely a fantasy these days, though...

Exactly. Bethesda does a really good job at producing DLC, value-wise. At least, they are for The Elder Scrolls series.
While I don't hate it, I'm kinda mixed on karma meters in open world games. While I like the idea that the game's people treat you differently if you're good or bad (or even change the story slightly), sometimes I just want to goof around the world, which is tough when you're trying to be good, out of fear you might accidentally hurt people.

Being forced to replay an entire large section of a game (anything that lasts more than 20 minutes, I'd say) just for failing once.
I hated that giant fan puzzle in Twilight Princess. You drop from the room above and there's no quick way to get back up to try again.
I MISSED? Well, sorry Link but I got to drop you into the pit. I'm not repeating the entire dungeon to get back to the door the intended way.
Also, Skull Kid's minions in the Sacred Woods. I'M TRYING TO SOLVE THIS MAZE. SKULL KIDS GO THE **** AWAY! I wish I could DELETE you from THIS GAME.

Those are storytime trees. People cut them down and use them to make Plot Armor.
Not being able to save where you like is a pet peeve of mine, especially in games with bad checkpoint systems. Such an antiquated console-y design that I'm surprised it's even still used in most games today.
QTEs in general, quick ones are fine, but there are games that exploit this to hell, having excessive QTE chains where a failure means doing it again or getting a game over. Played a level in Bayonetta getting gold medals after every battle on a first playthrough and died on a quick time event 7 times in a row before succeeding, earning me the lowest rank.
I share everyone else's sentiments about tutorials, they should be in a demo section in the main menu, not part of the game. Even worse when they force you through them in a New game+ mode such as in the first Dead Space!
@spizzamarozzi While GTA does usually hold my interest when driving due to passing places you can visit, I remember dropping Far Cry 2 because of this. Constant driving and bombarded with regenerating bandits in cars.
@CanisWolfred I agree with you in forced stealth sections. Stealth in general I don't have the patience for, but when the game suddenly decides that in an action game like Return to Castle Wolfenstein for no reason when you have been causing carnage for 95% of the game, it really irritates me.

Also when the game autosaves anywhere beside campaign. Basically if you make a choice in making a weapon or so and you want to change it, welp to bad! The game automatically saves, saying "**** you, you already made your decision!"

When you need to mash a button rapidly in order to beat a part of the game. Some minigames in mario party are guilty of this and i would always lose them >.<

Have you ever played the original MGS? I've replayed that game countless times over the years, and not once did I ever save Meryl. That stupid button mashing torture minigame was just annoying. Not to mention that you get the invisibility gear if you let her die, which is insanely fun to play around with.
