VVVVVV. It doesn't look that hard because of the checkpoints, but the Graviton, Time Trials (if you want to get V's) and No Death Mode are harder than any other game I've played.
VVVVVV is an extremely hard game as well as World's Hardest Game. Donkey Kong Country Returns was also somewhat difficult. Another game that was sorta tough was Art Style Light Trax.

You must be trolling. Super Meat Boy is fun AND hard, while Nintendogs is hard because it's boring.
The hardest game I ever beat was Super Ghouls 'n Ghosts on the hardest difficulty setting. It's not too bad on Normal, but if you play it on the highest setting it will really put you to the test. The hardest games I ever played but have yet to beat without cheating include Ghosts 'n Goblins and Battletoads.
