In the game, you type in what Mario game it would be in. The name is the name of the powerup. Description is what the powerup looks like. Effect is what the powerup does to Mario. Frequency tells how often the powerup appears throughout the game. Here's an example:
Name: Acid Flower Game: Super Mario Bros 3 Description: A purple, wilted flower. Effect: Mario can throw acid balls, which look like purple fireballs that will eat up blocks and enemies. Mario throws the balls straight ahead and they slowly fall down. Frequency: Rare, but not as rare as the hammer suit.
Name: Glue Mario Game: Super Mario Bros. 3 or Super Mario 64 Description: A very gluey mushroom Effect: Mario becomes very gluey and can stick to any surface. He can climb walls and stick to ceilings. You can't jump though, but when you run into an enemy he will stick to you like a katamari ball so you're basically invincible. But each enemy that sticks to you also slows you down which would make for some interesting levels where you actually have to avoid enemies in order to run quickly. But if a bomb-omb sticks to you, it can blow up and kill you.
Name: Ninji Suit Game: Super Mario Bros 3 Description: A suit that makes Mario look like a Ninji Effect: Allows Mario to cling to walls for about 10 seconds, allowing him to wall jump if done correctly. He can throw shurikens at enemies to defeat them.
Name: Glide Nozzle Game: Super Mario Sunshine Description: A plane-shaped nozzle found in green boxes. They shoot water at the end of each wing. Effect: These simply allow Mario to glide. They don't go as fast as the turbo nozzle but Mario falls much slower, making this useful for reaching far away platforms. This can't be used on the ground.

Name: Dragon Mario Game: Super Mario Bros 3 & World. Description: A dragon suit. Effect: Mario can now breathe a stream of fire out of his mouth. This is great for attacking enemies up close. However, breathe too long and Mario will get tired and he can't breathe fire for 3 seconds. Mario is also immune to fire attacks and can swim in lava. Frequency: Uncommon. Appears mostly in lava / mountain areas.

Name: Backpack Game: New Super Mario Bros. Ds and Wii Description: Looks like a backpack with the mushroom logo on it. Effect: When mario gets this he will be able to collect enemies by pressing the B or 1 button! When you press B, mario will do the same spin animation like he does with the cape. If you are near certain enemies (goombas, koopas, and other simple enemies) your bag will swallow it. The special thing is that if you have 4 or less enemies swallowed, you can fly like the cape effect! If you have 5-7, you will be able to jump on spiky enemies (not piranha plants though) without damaging your self. If you have 8 or 9, your jumps will be lower to the ground. When you have 10, you get a 1-up and the item disappears. Frequency: Very rare.

Donkey Kong Country 2 sorta had that. One enemy would try to steal your lives (another would take your bananas). But they'd float away as extra life balloons that you could try to get back.
