I don't know for sure, but try defeating every enemy in a chain. Also, certain bosses will give off medals if you slash them with your blade at the right time (such as when they come up close for a quick attack). If I'm being too generic, it's because I'm not the best at getting medals.
Yes, medals boost your score significantly. I haven't found all of them but some conditions that I noticed. Sometimes, you have to defeat a certain group of enemies/objects appearing one after the other (ex. the Machines that appear before the boss on level 0 or destroying all the oil drums on the grassland portion of level 4). Others, you need to defeat/attack a monster with a lot of HP within a short amount of time (ex. the Eel that gets stuck in level 2 has a medal). Also, there is one that I saw on the Nintendo Channel (Beating the boss just before the timer goes to 0). The last one are boss-specific Medals (ex. Using mines as projectiles in the incinerator boss fight in level 2 and getting an 8+ combo (I think) to the Comm. Deko fight). Those are some that I know so far.
it's a pretty safe bet that as long as you're killing every possible enemy, avoiding getting hit, and deflecting things with your sword wherever possible that you'll get a medal for it. there are too many to remember, but i can tell you that every level except the first gives you a few for reaching certain killcounts.
i know that in the first two levels there are a few for killing every tank using deflected missiles. you can get two from the turtle boss in stage 1 by hitting him in the face with your sword (it's only possible twice and you need full health), and there's another fun one to get if you can hit each of his knees with a deflected rock or a charge attack when he's raining metors on you.
you get one if you can kill at least two of those yellow eels that come at you in the tunnel in stage 2, and also if you blow up every glass building in the background in the first part of the level. you'll get one in stage 3 if you can blow up every segment of every one the hanging structures that appear about halfway through the level, and another one if you can destroy every one of the 'walls' of the boss shortly after, and another for successfully hitting the samurai girl each time she charges you.
stage 4 gives you one for blowing up all of those weird huts in the beginning as well as for killing all of those swarms of pink heads that come out of the windows early on. hitting the ball that the first boss tries to throw at you every time will also give one, and you can get one for blowing up every gas tank in the grassy field afterward by deflecting the boomerangs those red demons throw.
when you're fighting the giant frog you can get one by slashing his tongue before it hits you. when the giant slug near the end destroys the bridge and starts chasing you, there's a point where he charges you and can be hit with your sword; that'll give you one. the crazy shaman lady at the end will give you an easy one if you absorb one of the blue birds she shoots and slow time down.
stage five is nuts and the simplest way to put it is to kill everything. kill every tower along the road, every cactus monster that comes out of the ground, every stealth bomber, pretty much everything you can and you'll wind up with most medals in the level.
from there on i'm a little sketchy, so i can't help with the end much.
