A lot of people are going to disagree with this, but the sims I thought was terrible, one of the most pointless and mind numbingly boring games I ever played, I can't stand that franchise at all.
Skulljagger for the SNES was bad…ok, probably nobody ever played or heard of this game. It’s so bad that it does not even have a wikipedia article! It’s just a terrible and extremely hard platformer.
Also Bible Adventures is awful on the NES. This game just rips off the SMB2 game engine, but adds stiffer controls and horrid gameplay. In fact, this game had no Official Nintendo Seal labeled on it, making it an unlicensed Nintendo product.
@Terranigma: that was Takeshi no Chousenjou... i saw a video of a dude playing through it on Youtube (it was a younger comedian on a Japanese TV show), and it was hilarious to watch (him struggling through the game, not the game itself).

@TwilightV: i just checked that out on youtube, and... wow. that is a terrible game in and of itself, but you're right, the way he's insulted (and the way he whines, too) is pretty funny.

The Worst Video Game ever? I tell you what is the worst video game ever. E.T THE GAME!!!!!! THIS IS THE WORST GAME EVER FRIGGING MADE!!!!! It caused the video game crash of 1983, people didn't buy games anymore and it sucked. If it wasn't for this game, Atari would still be awesome but no. But our saviors Nintendo came and saved us with Super Mario and its cool light gun.
I'm surprised nobody has mentioned Total Recall for NES! I actually had the unfortunate honor of owning that game back when I was young. Fortunately I was able to return it to the store I bought it from back when stores actually let you do that! I would say that Total Recall started the whole movie to video game cash-in trend but I remember ET on Atari preceded it. Ah, horrible memories!
I forgot Polarium for DS. I thought it was good at first. Then I realized I was wrong. These are words you use to describe it: Drab. Boring. Pointless.

Regarding E.T. for the 2600, I don't think it was as bad as the rep it gets because of the crash. Granted I was only maybe 8 or 9 when I played it, but we had a pretty big 2600 collection (30-40 games) and I remember playing that one for hours on end more than many of the others. It's not a great game by any stretch, but at the time I didn't think it was awful either.
Regarding TMNT for the NES, it was extremely difficult, but I don't think it qualifies as "worst" either.

Of all time? ET. Of the ones I've played? Superman 64. Stil I dislike ET because of the land fill and the crash. Atari reps were so stupid to rush it, although for six weeks of dev time, isn't as bad when you put it into proportion. Possibly Pac-Man for the Atari 2600, considering how bad it was and how they made more carts of it that Ataris they sold.
