UNEXPLAINED health regeneration. I don't mind health regeneration when it has good reasoning behind it (usually based on plot or game world).
Deus Ex Human Revolution makes sense since you're an augmented human. Not to mention early game, if you run into a firefight you will vapourize (especially on the higher difficulty levels).
Shields in Borderlands 2 make sense since they're based on the technology of that world. The majority of them are also as frail as Adam Jensen, especially on higher difficulty levels.
A bunch of other games have situational health regen, usually explained by a special trait the character has (caps on the amount healed, very specific circumstances for regen to trigger, etc).
And then you have games like the CoD series where your health will regen to full health in a very short period of time, with no logical reasoning behind how it happens.

realism if I wanted realism I'd go out and do something, I have a preference for things either completely impossible and unexplainable or simply not of this world. realism is good as long as it stops at the "looks realistic" and not at the "just like in the real world"

Software updates galore.
While I don't mind bug fixes & stuff like that let ever since both the PS3 & 360 allow software updates to patch up any games it allowed developers to slack of because they now can easily fix any bug.
Beforehand developers made sure there games work with little to no bugs at all then now that they fix anything post release they now only provide 70% effort to release the game to retail then provide the fixes afterwards instead of doing it all from the beginning like they should from the start.
Rockband III is a pretty good example of all this as it was launched riddled with bugs.
Sure they released a patched, except that patch is unavailable for Wii owners due to lack of update feature.
Then to add insult to injury they said they can't fix it.
Apparently Activision didn't receive that memo because they are able to give replacement disc's to fix a minor audio problem with Guitar Hero III.
Even more recently Nintendo released a downloadable app that fixes Legend of Zelda: Skyward sword glitch that prevents you from progressing the game.
As you can see it show how slack devolpers become because thay can easily fix the game post release.
Sure I don't mind patching up broken games via update but the update also unfortunately allow developers to release faulty games more often which I don't like.
Yeah, don't get me started on updates. I'm tired of having to always update something seeming every time I boot up a game. Sure, being able to fix things is nice, but I should never need over a Gigabyte of just patches. That just shows how little you playtested before release, and it reduces the chance that I'm gonna buy your games day 1, if at all.
