This thing was talked up so epic sounding. Wow, an X-men RPG, decide, shape your destiny. Turned out to be a mediocre, repetitive linear action RPG with lame character cameos, subpar story, and one choice that meant squat in the end.
I have quite a bit:
-NSMB2
-Super Mario 3d Land (Far too easy, generic, and linear)
-Luigi's Mansion: Dark Moon (Still fun, but somewhat of a disappointment)
-The trailer of Zelda 3ds (Looks like Ninty got lazy and is just rehashing old ideas. I'll wait before really judging it though)
-Majora's Mask
-Paper Mario SS (For reasons that most people already know)
I have more, but I just can't think at the moment. Also, I feel like this is a good stopping point.
Sticker Star. It's not a bad game, but I can't enjoy it. It's just not Paper Mario. I'm seriously worried we're going to see similar stripping back of RPG elements in the Mario and Luigi game, so I'm trying to not get too excited for it, just in case.

I don't know why people think this. Most of Sticker Star's problems are pretty blatantly just the team thinking of interesting ideas that backfired.
As for me, I have quite a few game disappointments.
Bomberman Jetters - I really like the single player Bomberman games but I think this one killed it as it was literally Bomberman Generations again, except far worse.
Banjo Kazooie Grunty's Revenge - Banjo Kazooie: Nuts and Bolts is too cliched an answer. Grunty's Revenge was a game that was announced 2 years earlier and no one knew existed when it was released. I just happened to find it at Gamestop one day and I was so excited by this. But, the game, while not bad, could not hope to be even half as good as the N64 titles.
Metroid Other M - I wanted to love this game, I really did. Seeing a deep look into Samus Aran with a narrative heavy game was something I wanted to be brilliant but it just...wasn't. At all.
Yoshi's Island DS - I disagree with people's pessimism towards the new game but I can't blame them because this game was so weak. It just wasn't that fun and it was aesthetically boring in almost every way.
DBZ Legacy of Goku - I was 12 and dumb and a DBZ fan. The game sucks so much, you have no idea.
Viewtiful Joe 2 - I don't even remember why but this game disappointed me a ton and wasn't that fun when it came out. And I loved the first game.
Modern Warfare 2 - The irony of this is I went into Call of Duty 4 expecting to hate it. But I really, really liked Call of Duty 4, its a very well made FPS in most regards. At worst you could call it a good example of a lame genre of FPS I suppose. But Modern Warfare 2, while having some real highlights, was mostly just not that fun and trying way too hard to outdo the first Modern Warfare except forgetting to have good video game levels or even attempting to be anything beyond a ridiculous Michael Bay flick (except more boring).
Samba De Amigo Wii - Inexcusable.

^This. I got the game for Christmas but just can't get myself to pick it up most of the time. I think I made it to the end of world 1 and that's about it. That's really not like me with a game, ESPECIALLY a Mario game. Honestly the Mario Tennis for the 3DS disappointed me a bit too. I think they are trying to make things too simple in an odd way.

@kkslider Viewtiful Joe 2 disappointed me, too. It was harder than the first, to the point of being frustrating, in large part due to its often obtuse puzzles.
5. Devil May Cry 1. Good god that camera makes it literally unplayable! Thank god the rest of the series is so much better in that regard.
4. Too Human. Never again will I ride the hype train before I see the actual gameplay...
3. Half-Life 2 - RAVENHOLM
2. Every Level 5 game that's not Ni no Kuni. They're a blackhole where good ideas go to die in crappy games...
1. Star Ocean: Till the End of Time - the only thing that wasn't false advertising was the freakin' subtitle...
Skyward Sword not being as fun as I thought it would be.
Kingdom Hearts 3D, definitely. There were just... so many mistakes in that game. I definitely should not have gone into that game expecting an experience like Kingdom Hearts II.
It's a good game, but too flawed to be in the upper echelon of eShop offerings.

That and Mighty Switch Force. I like the game, it's just it gets relatively boring with the same mechanic over and over again. I dunno, I was disappointed with it. I like it but it's not a good recommendation imo.
I never had a problem with the 'toon visuals (I think it looks fantastic, actually) but the game environment is severely lacking....a huge world, but broken up into tiny little chunks separated by sailing, sailing and more sailing. And then more sailing. Yawn. The islands were too small to be worth exploring properly and didn't have much interesting anyway, the dungeons/bosses felt very short and easy and it just all felt a bit...'My first Zelda game/Zelda for Dummies' to me. That being said, I'll still give the Wii U HD version a chance.
