It happened to me as well. Back out and re-enter and do the survey. Then go to rewards and it should be there.

I just finished the first four levels. I'm liking the game so far! I feel that I'd definitely have to play in small periods, since I got tired of the button mashing after a while. I'll continue on tomorrow.
Edit: LOL the code is literally on back of the box art paper. I have to take the paper out just to read the code! XD
It took me a while before I just found it there. It was pretty weird, but I'm glad I have the code, I guess. I wonder if this is a new thing they'll continue doing... ._.

Wait, is there another code? Or is this from another retailer? These multiple codes thing did confuse me a little. x_X (I got the OoT one)

I wish the Legend scenarios weren't identical for all usable characters. I mean, take the Death Mountain as an example. If you play it as Impa, you do exactly the same things you would do as Sheik if you played as her. I don't mind the main flow of the events staying the same, but the way the actions are split between the player and the other character should be different.
That would be cool but i see why didn't do that. Then the player could just hang back while the AI (Link or whatever) did all the work. Also it would cause problems if the character wasn't supposed to be there at all like Ganon in the first stage

Aye-- I got that. Trouble is I keep/keot getting stuck/trapped in the area for long enough that the Drku Tree fell before I canget Gohma below 20%, let alone dead.
I got absolutely no sleep last night. I hate videogame hangovers. That being said this is a very solid title. I don't play enough Musou games to be considered a fan (I only like the samurai line....and not the main dynasty line) so technically I am the Zelda fan that bought this game. Gameplay is solid, fluid and this game feels like it can handle more enemies on the screen than Orochi 3. I love the characters and adventure mode is a blast...honestly I'm spending most of my time there. Music is great, I like the rock infusion that doesn't destroy the source material. Textures are a little muddy but since that is normal for what I've seen in a warriors game I'm not knocking it...the characters and enemies look great. I appreciate that there is a larger variety of enemies than I expected. Plus they all look like they belong in the game despite being from different Zelda games. Cut-scenes look great and I am so glad they went with no voices except the storyboards. I hope this sales well enough to greenlight a sequel.
Also, I would totally be behind K-T and Team Ninja making a Impa or Sheik/Zelda spin off action title a la Ninja Gaiden. I love Link but he has been bench warming most of this game for me.

Let me join in on the I stayed up way to late playing this last night and I'm paying for it now comments lol.
After an hour into the game I thought it was ok. 7/8-10 for me. After last night's play session that quickly jumbed up to a 9.5-10. There is just so much content that I can't get enough of it. I love how if one of your characters gets a new weapon it vastly changes their play style. I wasn't really high on link either like was said by @Pahvi but after using his fire rod I really like him now.
Also the adventure mode has that just one more quality to it that kept me up till 4:30 last night and I needed to be up by 9 so yeah I'm hooked!
So is the Epona DLC OOT or TP. Because if it's OOT than Epona will be useful for exactly one required objective and some side objectives, while TP Epona would be useful for the first half of a battle before being entirely forgotten.
Those historical infos are just 3 small text boxes which you can access in the tutorial. Just press Start ingame, they sometimes are shown during the loadingscreen aswell.
The 8-bit sword is first given to you as a reward for beating Legendary Mode. Afterwards you can get one as a regular drop, with better stats and slots.So don't worry about selling your first one, you can get plenty of others.
The costumes are changed by pressing X during the fighter selection mode. Just hover over Ganondorf, eg., and press X.
