Largely, Mario's last name has been Mario. But the brief periods in which his last name is not Mario — although no alternative has been suggested — are not insignificant. The most interesting one is the three-year period after which Satoru Iwata, former Nintendo President and CEO, stated categorically that Mario's last name was NOT Mario, but after Iwata's death in 2015, Shigeru Miyamoto returned us to the canonical Mario Mario.
Although, WAIT, plot twist: Shigsy actually said that Mario didn't have a last name in 2012!! THE CONSPIRACY CONTINUES.
Anyway. We all know that Mario's last name is 64, and Luigi's last name is Mansion.

All I know is Mario's parents let their babies get kidnapped and then left it up to wild dinosaurs to save them rather than contacting the police. Would you trust parents like that to give you a reasonable name?

Is their last name patronymic? Like Mario Marioson, Mario McMario, Mario Mariovich.
Their last name is definitely not Mario. They are brothers and one of them is named Mario. That's the explanation. It doesn't have to make sense because it was made by people for whom English was not a first language.

It’s Mario and has been since 1983. Mario and Luigi are collectively referred to as the Mario brothers.
Doug Walker and his brother Rob Walker would be collectively referred to as the Walker brothers, not the Doug brothers.
(I’m using him as an example since he caused a stink about the name years ago.)
Warner Bros. is also another good example. It was founded by four brothers from the Warner family and that’s how it got its name.
The Mario movie didn’t invent or confirm the surname. A name in front of “Bros.” makes the name the brothers surname. That’s just how it works. Now it’s possible that Miyamoto did not realise what calling the game Mario Bros. would do, and back in 1983 Mario’s popularity wasn’t anything like it would become later. But really that’s how the naming structure works.
I just always thought this last name was Mario and was known by that, whilst Luigi was Luigi Mario
