I guess I'll say Kingdom Hearts: Chain of Memories.
It wasn't a bad game, and there were some very cool things about it, but I'd rather have had them use a normal ARPG battle system, instead of turning it into a card battle system.
Also, even though it kinda makes sense given the game's story, I was a bit disappointed to discover that the stories of the Disney worlds were the same as the first game (just replace "heart" with "memories" in all the dialog). Oh, and then there was the fact that a room's appearance & function depended on what type of card you used to open it (which I didn't realize until late in the game).

Yoshi's Island DS, for being just a disappointing sequel to the original Yoshi's Island. Not bad technically, just doesn't live up to the original and hence disappoints me more than a lot of other gaming sequels that I've played. Feels more like a half done ROM hack.
New Super Mario Bros 2 as well I guess, although that's more it being an unoriginal '7 or 8 out of 10' experience rather than it being legitimately awful. Good game, bad sequel considering the games it came after.
I haven't played too many utterly terrible games really.

It's so hilarious to me seeing people list Super Mario Galaxy 2 as the worst sequel they've ever played. It's alright if you didn't like the game that much, but worst sequel ever? You must not have played a lot of sequels.

fourth! it wasn't that bad, I must agree the stage selection was lame, for a 3D Mario(and sadly it was repeated in SM3DL), but that's not a game breaking problem, just a matter of preference!

Yoshi's Island 2 is my least favourite, simply because of how great the first is. But worst sequel ever would have to be SMB2. The level design is bland and soulless, even the final boss is clearly a phoned in effort. Garbage in my opinion and my most regretted purchase.
By the way, Galaxy 2 is amazing! Who gives a crap about the hub world being bland. Mario games are about the levels - and that is why Galaxy 2 is a master piece.
Honestly, I hope the next 3D Mario forgoes levels and just has a massive overworld, like Jak & Daxter had.
^^^^^^^one million times this(or at least something like Banjo-Kazooie a big overworld with big levels inside and not binding the player to specific missions in specific orders)

I just don't get that. I mean, maybe the presentation was bad(I don't agree but let's say it is for arguement's sake), so what? Did you not enjoy the game? Did you stop right after the first couple levels? I'm just not quite understanding how that destroyed the game for you.

Ummm... The hub world is very important, One of the things that made 64, Sunshine, banjo-kazooie, and others great was the awesome hub world with places to explore and secrets to find. I think the comet observatory is a bad example of a hub world, i mean its there, but there is nothing to do. Also, the game i would most compare SMG2 to is 3d Land, not Galaxy 1. Most of the levels in SMG2 are soooo straight forward it's almost not even the same type of game anymore. It was unbelievably easy, and I'm no speed runner, but I beat it in under 9 hours, without green stars, which were just added in to try and make the game seem longer. The new power-ups added nothing to the game.I know it's supposed to be the leftover ideas from SMG1 and clearly they used the best ones for the first game. Also Yoshi is advertised as something new, but he is used in very few levels. It just felt like a watered-down version of the first game. That's not to say it's a bad game, but It was disappointing, not as good as the others, and certainly not the best game on the wii.
The most disappointing sequel I've ever played has to be Epic Mickey 2. It just felt so off, like they wanted to make a big budget game, but forgot what made the first one work.
