Devil Survivor never really showed any of that, though, and come to think of it, neither did 999 when it came to the uber-gory parts (i only mention 999 because it squicked the hell out of me, ymmv of course). Those games were more psychological in terms of how twisted they were; it required a bit of thinking to figure it all out. This one is very up-front and personal with it, however couched in cute-style drawings it may be — and that brings up another point, kinda... since it's so 'cutesy', will Nintendo be willing to release it and then deal with parental backlash once they realize exactly what kind of game they just bought their kid? (granted, that's in no way Nintendo's fault, but you know it's gonna happen, lol.) Devil Survivor and 999 were both pretty grown-up in terms of looks, gameplay, and reading comprehension required, but this has the at-a-glance look of something far less disturbing than it is when you get down to it, and it doesn't seem like the kind of game that would require someone to think really hard about what's going on in order to get through the game.
I think at the very least it'll be interesting to see if and/or where Nintendo chooses to draw the line in terms of their in-house censorship. Will they allow this game to be released as-is, will they require some graphical changes be made, or will they not allow it to happen at all? :3

Nice, they'll add the expansion. Depends on what gets cut, but I may get it just to support more indie releases on eShop. It took awhile, but I've decided to do the same with VVVVVV.

Honestly, i can't see Nintendo possibly green-lighting a game where you're a child trying to escape from your mother who is trying to kill you, which features a dead child locked in a chest as one of the endings.
After much debate Nintendo has decided to release "the Binding of Isaac". There have been a few minor changes for censorship and to make the game more palatable for younger players. This is what the game will now look like:
Honestly, it's better this way, lots of parents who ignore ratings, have no idea of how to use parental settings and/or can't control what their kids play would've eventually come up with stupid complaints about the game only because it is on a nintendo console.

Really bummed about this, Isaac was one of 2011's best games, IMO. I wanted to play it on a system, now I have to go back to playing it on my sisters computer.

they'd still have to get it ESRB rated, though. weren't they complaining about that for Nintendo's approval process?
edit: actually, here's a quite relevant article about this game and its potential rating as a barrier. if it scored an AO, it'd be a no-go for Sony as well (unless they've since relaxed their policies regarding third-party AO titles).
