So I played A Link Between Worlds and Monster Hunter 3 Ultimate on the stock 4gb SD card. I later bought a 16gb card (from Nintendo's online shop) that I want to get all of my games onto. I obviously can't format either card, as I would lose hundreds of hours of progress.
I just want to ask; is there any way to migrate the data from the 4gb sd card onto the 16gb card without losing the contents of either?
Place one SD card in the north and one in the south. Wait till winter, when data flies south to escape the cold and trap the free data in the SD card you put in the south. But seriously, you should be able to copy the data and save it onto another SD card with your computer.
You can try to combine them, but it would be plain easier to just overwrite it
This is how I usually do it:
1- put freshly formatted card in the system and allow it to create the starter files
2- copy everything from the old card to the new one, but don't overwrite anything when asked
3- use the new card
4- ???
5- Now you have more space and one useless SD card.

PROFIT!
OP, I recently transferred all of my data from the stock memory card to a brand new one. I'm sure if you just transfer your old files onto the new SD card it should work. If not, you may have to format the new one. Remember, backing up files saves lives.
Oh my God I read that as "SD Card Minimum" and my head almost exploded. O_o
Anyways, question: what if your computer lacks an SD card slot? I mean...my new comp has one, but I old one didn't and I could never figure out how to do it with just a 3DS...

Buy a reader or get one bundled with the new sdcard.

Freedom of the press is for those who happen to own one.
Having done this twice, from a 4GB to an 8GB, then to a 16GB, this is what I do:
1.) Take out the old SD card from your 3DS (with the power off of course)
2.) Plug it into your computer and copy ALL the files into a folder on your computer.
3.) Plug in the newer, bigger SD card into your computer and copy all of those files onto the new card.
4.) Insert the new SD card into your 3DS and turn it on. It should work completely fine as if nothing was changed. That's pretty much exactly what happens, except now you have more space to utilize!
