Honestly, and others who have played the game can back me up on this, the hardest game I've ever played (and quite possibly the hardest game ever made) was F-Zero GX. I'd like to meet ANYONE who could beat the story mode in that game, even on the easiest difficulty setting. If anyone reading this has done so, I salute you!
My brother has. I don't know how he did it. He's not normally that good at games.
@JaxonH: I've done everything but the Chapter 7 on Very Hard difficulty. The key to beating it is to have a lot of time to just keep repeating the mission until everything makes since and you get a bit lucky. It took me entire days, sometimes multiple to beat some of the later challenges.

Battletoads or Ghosts N Goblins....played these but never got too far. I don't think ever beat the rocket sled sequence with the barriers in Battle toads and the other I maybe made it to level 2 a couple of times
I did beat Mega Man 1 and Castlevania with the help of my NES advantage turbo and slow mo features on the original console and I can beat Ninja Gaiden 1 and I think I beat part 2 but never played 3 much at all.
Metroid nes. I can't get past kraid's lair. Not having a map is extremely frustrating. I prefer the vastly superior zero mission gba remake. In fact, zero mission is actually my second favorite metroid game! But metroid nes is terrible.

I know it's cliché to say this, but Dark Souls is up there. I'm only about 8 hours into my first ever playthrough, so I suppose I can't really comment on it too much because I haven't beaten it yet.
The story mode of the Twisted Metal reboot for PS3 was ridiculously hard. My fiancée and I had to play co-op in order to get through it.
And I know this one isn't necessarily known for it's difficulty, but Metal Gear Solid 4 was one of the most stressful games that I have ever played. Those boss fights...
As for Nintendo titles, it's probably Mario Galaxy 2.
My game library ranges from Mario Kart to Call of Duty to Tales of Xillia to Diablo 3 to Forza. To me gaming is about having fun - not blindly supporting only one company while thumbing your nose at "the evil other systems."
Battletoads on the NES for me. I wish there was any chance of the game getting released on the Virtual Console (there isn't, because Rare), just so I could give it another try.
For me it was Donkey Kong Country. I was young when I got this for my birthday and it took me 3 yrs to beat that game. I almost broke my SNES controller.
Dude, you are not alone on that one. When I was a kid I was stuck on the Poison Pond level for 9 months! The day I finally beat that level was the day I beat the game for the first time. I can now plow through the game in less than an hour, but that level still makes me nervous.
In fact, Cranky saying "I beat this game using one life and it took me less than an hour" during the ending is what has always motivated me to speed run this game. My personal record is 52 minutes, with 5 deaths. I know that's nowhere close to an actual record, but it's the best that I can do

I am thinking I might add Castlevania 3 Draculas curse....I never came close to beating it on the original console, I only played it at a friends house but I have it on virtual console and judging how badly I have started abusing save states to get through this I can't fathom the level of mastery required to beat this game properly. (I been only using trevor belmont because that wizard girl seems worthless to me)
Castlevania: Rondo of Blood is a tough one. I played it at my brothers house and I couldn't beat the second level. Super Mario Bros: The Lost Levels, Zelda 2, Advance Wars: Days of Ruin, DKCR Wii, I'll come up with more.

Fire Emblem Shadow Dragon. This is the kind of game that gets hard because of the expectations you put on yourself, like not wanting to lose any characters or wanting to level grind before progressing at certain stages. It's a hell of a challenge to get what you want and progress in this game.
