Well, Hard mode in Mario & Luigi Dream Team is utterly brutal. You've got enemies doing 14 times the usual damage, a hard cap of items in the inventory and far, far harder giant bosses than in the normal game, and that's before you get to the Giant Battle Ring. Five bosses, no healing, no continues, about 10 attacks a turn (for the enemy) and they kill you in about two hits. Add how they're marathon bosses with often flawed gyro control sections, and I'd honestly bet anything that most people would have more luck beating worlds A-D in the Lost Levels than trying to stay alive during those battles.
Maybe parts of Yoshi's Island DS as well. Starts easy, gets insane about world 5 and ends up being the closest thing to kaizo by the secret levels. The game actually hates the player more than the Lost Levels at some points, with some required tactics being close to glitch abuse.
Either Super Mario Bros 3 or Mario 64. Both require a bit of aclimatising to the weight and control of the character. Whereas most others I can just pick up and play, no probs.
I really can't see anything being harder than Super Mario Brothers 2 (Lost Levels.) I've played it before and couldn't make it past World 3.
The original SMB is a walk in the park by comparison.
Guess you've not played many Mario games that aren't platformers then. Quite a few of the spinoffs make the Lost Levels look like a cakewalk...
And which ones would that be? I've been around since the NES and I don't remeber any Mario spinoff being even nearly as hard as Lost Levels, let alone as hard as to make LL appear like "a cakewalk".

Mario Kart Super Circuit, unlocking the night sky title screen by getting 3 stars on every cup for every difficulty is brutal due to the not so great drifting controls and needing to get virtually every coin. In terms of platformers, probably Galaxy 2 as it's the only Mario platformer I haven't fully completed (solely because of Grandmaster Galaxy's "The Perfect Run").
