Brawl, me and my friends played it almost every day after school when it first came out
Also, when I was younger I put about 370 hours into Pokemon Emerald, and probably a similar time for Leaf Green before I lost it...

It's always the newest Halo for me, so right now Halo 4. I also go back and play Super Mario World quite a bit, 21 years later and I'm still not tired of that game.

Super Smash Bros series - When I just want some mindless fun, I pop in a Smash game & go at it.
Kingdom Hearts series - I'm someone who has to see a story through to the end, so I'll keep on buying KH games until the story is complete (oh, and I do enjoy them as well).

These are probably the games I've played most that I've failed to tire of. (In no specific order.)
I know I probably have more, but they aren't coming to my mind right now.

Paper Mario and Smash Bros. Melee and Banjo Kazooie. Can just play those games at any time for no reason and just enjoy it. Same with my VC NES collection on both Wii and 3DS (boring games like Nintendo Golf and Yoshi aside). And Sonic 2. And Yoshi's Island. And Star Fox 64.
Smash Bros. Melee is definitely my most played game so that fits.

3DS: Kid Icarus all day son. Seriously, it's preventing me from playing the other games in my library like Kingdom Hearts, Layton and Revelations.
