Topic: Club Nintendo Survey for game you did not play

I use Club Nintendo for many years and I know I been putting in codes ever since the Gamecube and GBA era. This is the first time I have ever seen a game in my "To-Do-List" that I have not played before. So I decided to check my eShop account activity to find that game then I realized I never bought it or redeem it at all. So did anyone have this problem before?

Castlevania 2: Simon's Quest is the survey that I did not own nor play.

So how I report the bug to Nintendo?

EDIT: Okay guys, its turn out to be Donkey Kong Jr survey with Castlevania 2: Simon's Quest image. And thank you all for the great help here. Now I can answer the surveys with honest.

Answer the survey and tell them "i did not buy this why is it here" in the "tell us what you thought of this game" part.

A couple times I've had a game appear in my to-do list that I didn't buy, but when I clicked on it, it was actually a survey for a game I had purchased. It was like it just had the wrong image appearing for the survey for some reason.
