Believe or not, for me it was the original PC (not the oldest one but the second oldest that wasn't number two) Oregon Trail. That game scared me HALF-TO DEATH when I was little. I was always scared out of my mind that the wagon was going to catch fire (the music that played only for that event gave me nightmares). And the bad events in general with the "da, dun dun" theme kept me from playing it with the sound on most of the time. Then there were the pictures... One image that stuck in my mind the most was the bad event when one of your oxen died. One would think that I would be more traumatized when a person died, but they tended to drop like flies anyway. And at least the actual body of the person was not shown. Although the sick event image did scar me as well. I don't know why, but a lot of things from that game just made my skin crawl. Yeah, the things that scar you when you were a kid...
Oh, and Doom 3 (although I was no longer little). SPOILER ALERT!!!
I'm terrified of spiders and there's a spider boss in the game. I honestly ended up having a friend come over and I just shut my eyes and he told me where I needed to shoot in order to kill it. Not my finest hour...
The eel in Super Mario 64 freaked me out the first time I saw it. Just swimming down... "Oh look, a sunken ship... OH CRAP!".
SHODAN in System Shock 2 was pretty terrifying because of her voice.

Resident Evil 2. The game started and I couldn't even get to the first door.
Well it wasnt that bad for me but it definitely scared the poo out of me. I remember the first time I played this game I was spending the night at my friends house and we rented it. For some crazy reason we had the bright idea to play the game with the lights off during the night. So yeah we didnt get much sleep.

There was this Scooby-Doo computer game that my friend and I used to play in my mom's classroom after school. There was this one part where something happened--I think it involved a mummy--and you should've seen my face.
The next video game after that to scare me was Puzzle Agent, I think, so yeah I don't play a lot of scary games.
The funny thing tho is that I never got scared of this game when I was a kid,actually loved it:

I couldn't finish the level for the longest time because I was afraid of the eel. Now I go back to the level (age 21) just to listen to the music sometimes. Dire Dire Docks is just the perfect relaxation song.
I'm upset they didn't use it at all in the Galaxy games or in Sunshine.
