There were about 4 games that were the ABSOLUTE hardest for me, one is a hacked ROM called Super Mario Forever (if you don't know what that is, then does Super Mario Frustration ring a bell?), Demon's Crest for SNES (a masterpiece of a game though), Silver Surfer on NES (no words can describe it's difficulty), and finally, Dragon's Lair for NES (don't get me wrong the game SUCKED really bad, the controls were SO crippled, delayed, and stiff, and of course, the difficulty).
Mega Man X3. Sigma's final two forms were brutal torture, you wouldn't believe. Magician Lord. I couldn't survive a minute without using up a single life, and I got through half the continues to beat the game.
I'm a softie, so I don't play hard games (I like to, you know, actually be able to get through the games I've purchased ), but the one that comes to mind is the first Mega Man Zero game. I've been playing Mega Man games for a long time, so I know what I'm doing, but that one had me grinding my teeth at moments.

@StarBoy91: Ouch, Mega Man X3 and Magician Lord, those are very hard games, but I am a Mega Man expert, so X3 wasn't too too hard for me, but it was very challeging, then of course I beat the game. Magician Lord I have on SNK Arcade Classics Vol. 1 on my Wii, and it is freakin HARD!!
ikaruga is definitely up there for me, but really, it's all pretty pattern based, so once you learn each section, it's not too bad. and the longer you play the more continues the game gives you, so if you keep at it, you'll eventually win. it's still a booger, though.

I'm not gonna say Toy Story for the SNES, since it's more of a normal-difficulty based game, though it does get hard in the end. I've actually beaten it once without enabling the Invinicibility Code. Mega Man X3 was a challenging game, and I've beaten it on my last life. I actually almost broke down in tears because of how many lives I had lost. I didn't want to lose all my lives, since I knew the fate if I did (I'd've had to start Sigma's Fortress AGAIN).
Magician Lord is hard because it was designed to steal your money in the arcades, so the difficulty comes off as cheap rather than balanced, yet difficult. It's also not hard since you have unlimited continues, and you respawn where you die, so you'll die a lot but there's no real difficulty, it's more of an excercise in tedium.
Castlevania III is a really hard game, and F Zero GX has an excessively high difficulty level, however both of them are designed really well and are exceptional games.
Some people say Arcana is hard. I can kinda understand why that is, although I found the game to be quite easy (I have beaten it once). Arcana is a good first-person RPG, not hard, but it has a decent difficulty. I do love the random battles and the complex dungeons and the soundtrack.
Streets of Rage 3 is a hard beat'em up. To be quite honest, I've only beaten it a few times on Easy Mode (I barely play this game with other people, though maybe I should). It's a decent, but not great, beat'em up. But if I'm using the word "decent" wrong, please tell me.
Super Mario Bros. The Lost Levels is pretty tough, too.
Gah, that game makes me want to kill myself. D: It's like self-harm, it is, what that game does to me...I have vowed never to play it again.
