Agreed with Corbie. We talked about that the other day. Some really tough puzzles in that game, and often times interconnected with the entire dungeon.
I would also say that on PS1 Beyond the Beyond. Yes it's a turn based RPG, but one of the toughest I've played. Boss fights are probably 4-5 times as long as most RPG's, which can deplete your inventory fairly quickly. It also has some really long dungeons as you get farther in the game with some very aggravating at times puzzles. Also with that said though if anyone is considering this and is very keen on graphics, this is one of the worst visual appealing games on the PS1. The overworld is in 16-bit with only the battles in 3D. The battles still aren't pretty though. Then again anyone playing from this generation is probably not too concerned with graphics if they are going back to the PS1.

Fire Emblem: Radiant Dawn is by far one of the hardest (if not, then the hardest) Tactical RPG's ever brewed by the devil himself. Said game is very difficult on Normal- in which you have the ability to save during a battle, then keep playing. On hard mode, the aforementioned ability to save mid-chapter is taken out. Also, you loose the ability to see how far an enemy unit can move, thus making winning strategies much harder to create.
Phantasy Star II is the only one I can think of right now. I made it fairly far in the original Phantasy Star, but I couldn't even make it through the first dungeon in the second game. I eventually just gave up on it.
Secret of Mana is not tough though. You can spam magic on just about every boss or common enemy.
Shut yo mouth, foo. I died plenty of times in Secret of Mana, with or without magic, and I know others who have, too. The ordinary enemies alone can kill you pretty fast.
Haha that happened to me ALOT. So much so that I've (temporarily) given up on that game.

Anyway, hardest I've played was Xenosaga Episode I. Super slow and super hard combat. Even random encounters could spell death and the loss of an hour's worth of progress. Good thing it had a great story.
I've also heard that Demon's Souls is ridiculously difficult.

they're strategy RPGs, but try beating any Fire Emblem game without losing a character. that takes as much skill and strategy as chess, and possibly more so, depending on the difficulty level, given that each level brings new challenges instead of the same square board and same pieces in the same configuration.

I'm playing through Secret of Mana for the second time right now, and I've got to agree with Vay: it's not really hard. It just requires lots and lots of level-building (not just with the characters, but with the weapons and Mana spirits, too), and you'll survive longer if you've got the most powerful equipment with you. So far, I've gotten far less game overs (I've only lost a few this time) than I have on my first playthrough.
Maple Story. Which is an MMORPG, but hush. It has 'RPG' in the genre name. It counts. Sort of. The challenge is trying not to die of boredom. And repetitive strain injury. I got halfway through the tutorial, it asked me to kill 65 slime with my pathetic bow and arrow (on top of the 40 it had already asked me to kill) and I just thought "yeah. Screw this. That drying paint over there isn't going to watch itself."
Which is annoying because it's the only MMO I've ever encountered that I've liked the visual style of, people I know play it, and it doesn't have a monthly subscription fee. I just can't hack it with MMOs xD
