I forgot to save when I played Pokemon Ruby.
Like SO MANY TIMES. It was painful.
More recently I always forget to save in Fire Emblem: Awakening, before I started a battle.
I always get scared because in the last battle my character broke their sword so they can't attack and I totally forgot.
Then I fail.

I forgot on Yellow version like 3 times in a row lol, I had to go to sleep to make sure I didn't flip shit
Zelda games are the worst when you don't save them! Happened a bunch of times with Ocarina and Twilight Princess for me. I can never remember to save right before the final boss either so that when you finish it the game always returns to the last save point (which is usually near the start of the final dungeon or Ganon's Castle or whatever.) So that makes replaying the last boss all the more annoying.
My worst experience however was after finishing Wind Waker for the first time, it asked me where to save the new second quest to and I interpreted it as it asking me to save the game after I had finished it (and I was like FINALLY a Zelda game lets me save after I beat it!) Much tears were shed when I went back to the title screen and found out what I had just done to my save file I'd spent the last 3 years playing... It really killed my interest in playing the game again until Nintendo announced Wind Waker HD so I can replay it all again with it still being fresh.

I've never had problems with forgetting to save, but ironically I restarted Pokemon Diamond and got all the way past the 1st gym only to realize I forgot to erase the previous file.-_-
Probably the worst thing of all is booting up a console to find all the save files on your memory card have suddenly vanished. Happened to me when I went to continue my current play through of Pokemon XD, it was so disheartening I didn't even feel like playing anymore.

I've never forgotten to save. Saved in the wrong place/wrong time? Definitely. Accidentally had the game lose power while saving? Yep. But I don't think I've ever forgotten to save after a long period of playing a game.
I don't often end up not saving my game.
I usually forget if I saved, so I do it again... Then I forget if I've saved or not... I usually do that a few times, and when I'm sure that I saved, I turn the game off.
lmao i do this all the time. So many times i cant remember if i had just saved or not. Mind you this was about 2 seconds ago with no distractions.

There is a huge amount of time after a certain point where you cant save. I left my Wii on for quite a while that day with a couple of breaks until i finally finished it off.
I've spent far too long with a Game Boy, or a DS, or something else running off batteries (in some of my older consoles prone to falling out), without nearby power source/spare batteries, to forget to save. It's pretty ingrained into me by this point.
New area ahead? Save.
Important battle? Save.
Defeated opponent in important battle? Save.
Made some progress? Save.
Potential for some unknown trap ahead? Save.
Pokémon center? Save.
Battery light red? Save. Save again to be sure.
Got momentarily distracted, not sure if just saved? Save again.
Battery light flashing? Save, and then
God forbid I lose the 20 minutes' progress since the last save
I've lately gone crazy and started saving things only when I absolutely have to. I self impose challenges like these on myself for no discernible reason, all the time.
Consequently, I've been having some trouble in games without checkpoints and autosaves, lately.
In Twilight Princess, I kept on getting to the Desert and beating it... then my brother gets the urge to start a new save file. Oh that's Dad's, don't delete that, Oooo! This one! 6 TIMES!
I'm not saying let's kill all the stupid people, I'm just saying let's take off the warning labels, and see what happens.
