I keep my system set at three rows. This gives fifteen icons on the home screen. the icons are not too small, and it doesn't take too long to scroll to the other side. I arrange by columb, and I have a whopping 96 icons currently, a boatload of DSiware (unfortunately, I can't buy any more DSi games because my system memory is stuffed - Ninendo, please make it so we can run them from SD ), including all the ambassador games, and every free preview video ever released!
If a particular catagorey adds an additional icon and there is no room to fit it, then I move every item entire in the remaining inventory over one space to the right. That leaves two empty buffer spaces for additional downloads before I have to shift riight again.
My first 12 items (4 columbs) are the default preinstalled apps. The only rearranging I did was to swap the shop and game icon so that the game card is always the upper left, like on the Wii.
Columb 5: Netflix (I also have Netflix set up on my Wii & PS3), Nintendo Video, and Nintendo Zone (it installed itself on my 3DS at Best Buy - it's useless but I won't delete it because it'll probably just reinstall itself if I go back in the store, plus I'm a packrat that loathes the though of losing an icon. When Christmas time comes and I install the MK7 channel, it is going into slot 5c and Nintendo Zone will be kicked all the way to the right-bottom edge of the menu)
Columb 6: 3d classics: Excite Bike (free when it was first available), Twin Bee, and Kirby
Columb 7, 3DSware: Pokedex (useless, but free), Pyramids, Pushmo.
Coumbs 8-11: 10 DSiware games, my favorite of which is Shantea. Also includes freebe Four Swords, which I haven't tried yet. Two blanks unused in 11bc.
Columbs 12-14: All nine Game & Watch DSi games.
Columbs 15-18: Ten VC Game Boy Games. Mario titles are always first, third party games last. Two blanks in 18bc.
That's it for the black & white games
Columbs 19-25: All 21 Ambassador titles. 10 NES + Certificate + 10 GBA titles. The certificate takes the center block between NES and GBA. Very nice symmetry.

Columbs 29-32: Twelve moree videos, All 10 e3 videos, in order of interest + Green Goblin trailer + Shrek Thriller (yes, I'm a Shrek fan)
Columbs 33-34: Non-game DSi Apps. I don't use them much so they are last. Mario Clock/Calc (mostly useless), Insrtment Tuner (I own a guitar), and Scientific Calculator (pretty darn useful if I get stuck somewhere without my TI89 graphing calculator - I'm an engineering student)
Columbs 34bc-40: 20 empty spaces. Add the 4 empty buffer spaces and you get 24 unused tiles.
120 - 24 = 96 occupied tiles in the system menu: that's 80% filled up! And I can honestly say with pride that so far I have downloaded and kept every single free item that has ever appeared in the NOA 3DS eShop!
BTW, what happens if I stuff fill the system menu solid, and attempt to intsall/download the 121st item? Also, if anyone can boast that they have more icons on they're system menu, please post them here.
It's not particularly organised but I try to keep things like the camera, mii channel, settings all grouped together....and the games all grouped together.
System is set by 2 rows. Built in applications have not been moved, then 3DSWare (applications like Excite Bike, Nintendo Video and Pokedex 3D), Virtual console (NES then GBA, for now but when I get more I will place then in console release order), DSiWare and then 3D trailers sorted by Old then new. Oh, and everything is in alphabetical order by name.
I have it in four rows (tried six, but is too small for me), and the software is grouped by type (NES, GB, GBA, DSiWare, 3DSWare, Trailers, etc). You know what the 3DS menu needs? Folders. I hope Nintendo realizes this some day.
i just made a new layout for my menu, because before it was kinda chaotic.
first are the apps and the cardridge and build in software + excitebike 3Dclassic placeholder of 3 lines then the gba games placeholder of 3 lines then the nes games placeholder of 3 lines then GB-games (only two right now) placeholder of 3 lines then all my dsiware games (12 i think) placeholder of 3 lines then all the 3d videos from e3 and that 3ds-conference placeholder of like 10 lines then at the very end stuff that i never use like the sound-app, healthty instructions, ambassdaor certificate

Apathetically running top to bottom so going down it would be a,b,c,d kind of set-up, then the next row does the same de, f, fi,g as another rough example then I pick what screen size has the last spot all filled in. If you put in back to the single column it all in one straight apathetically.

I have the health advisory placed as far right as possible. The camera stuff, along with face raiders goes far right as well. The first column is mii maker, and some of the stock software that came with the 3DS. Then I have the cart,my 3D classics, and DSi ware in the next column. From there is goes GBA, GB color/GB, and then finally NES. I'm zoomed out to the second last setting.
